Templates, races, and overall passive effects included, what are all the ways that one could obtain a stacking energy drain attack? Preferably ones that can be used more than a few times a day, but overall, how could this style of combat become optimized?

Hm, and I know that there are certain weapons (I think in the DMG?) that will allow you to make energy draining attacks, but at a cost. It's somewhat disappointing that such an amazing attack form is so off-limits for player characters... at least if you want to stay competitive.

Soul Eater PrC, Book of Vile Darkness. There's some argument about whether energy drain can be used for multiple attacks in the same round.

There's also an Energy Drain armor/shield property in Defenders of the Faith that was absolutely insane... 1d4 negative levels whenever you shield bash. But then it was reprinted in Ghostwalk, and errata was issued to limit it to once a day.

Since not a lot of people remember Ghostwalk or that it got any errata... You might be able to sneak the DotF version past an unwary DM. Or there might be some wiggle-room with the errata... You could try arguing that the 1/day limit only applies to the shield version.

Speaking of energy drain, does vampire monk's flurry of energy drains work?

And if not, is there a feat that makes it work?

No, because a vampire can explicitly only use it once per round. (It's also an Su ability with no stated activation, so one can assume it's a standard action to make a single attack that bestows 2 negative levels + regular unarmed/natural attack damage.)

Shapechanging into a succubus is easy, or just be an Illithid Savant and eat one. Then grapple.
