Question #1:
.....Would it cause an imbalance to allow TWFers to get their off hand attack when they move, in addition to their standard attack?

Question #2:
.....Would it cause imbalance too compress the ITWF (grants an additional atack at +6 and +11 BaB) tree into one feat as opposed to ITWF, GTWF?

Question #3:
.....Would it cause imbalance to combine the Double Slice feat and the Two Weapon Rend feat into one feat, granting rend at +8 BaB?

Also a side question, does the attack granted by TWR apply things like sneak attack or smite damage?

Answer to side question: No Rend is merely extra damage

As has been implied, but not directly stated, the answer is pretty much always going to be no. Melee combat, and especially poor forms of melee combat, are far enough down on the totem pole that you can do just about anything you want to it, and not introduce much in the way of imbalance. Sure, things might go wrong if every TWF attack produces a spell that's at the appropriate spell level, but short of magic, things'll be fine.

Anything that improves melees (esp. not the "Thog use Twohanded Sword for 2d6 + 6934892574") is fine in my books.

e: This is not full fix btw. You should think about reducing the requirement of "Hammer the gap" from BAB +6 to HD 6 or even less. It is a VITAL feat for two weapon fighters.

TWR not but you get bonus damage from smite evil for off hand attacks, do you?
