New Table Questionnaire (WIP)

So, I'm a very officious and efficient person, and I often think to myself, "If I were joining a new table, or running a table with new people, how could I easily get to know the tastes and wants of the table/new players in terms of how games are run?"

Being a busybody, I had the solution: A convenient, multiple choice questionnaire to be filled out!

I have decided to make that questionnaire, and share it for the convenience of the forum. I welcome any suggestions for questions to add to it, in order to give a more holistic, complete perspective on any potential player/table.

So, any suggestions? Or do you feel that the entire premise is incredibly, inherently offensive?

Disclaimer: I advise against actually handing someone the questionnaire and saying "fill it out," as this might be seen as cold, impersonal, and offensive. Asking someone questions by way of an interview is generally more social, and gives you a feel for the person.

This is certainly a good way to get certain expectations out of the way. Although I'd prefer to have those sort of things in a gentleman's agreement type thing rather than in writing. Since presenting somebody with the need to make rulings on stuff at that quick a juncture can create snap judgements that may not be well thought out.
