Noob question, Does this feat give you an extra attack, or is the slam attack in place of your normal ones?

It's a natural weapon attack, and it doesn't specify secondary or primary. I don't think you can use it with a standard chain of iteratives, you should be able to use it in a full-attack with other natural weapons though. It also says "if you have a natural weapon use whatever damage is better" not specifying whether or not that should be a slam as well. Wow, that is one of the most poorly edited feats I've seen in a long time.

Would a character with Two weapon fighting but not multiattack get a negative to hit with that extra natural attack? (full attack action) if so, how big would the penalty be?

Some creatures combine attacks with natural and manufactured weapons when they make a full attack. When they do so, the manufactured weapon attack is considered the primary attack unless the creature’s description indicates otherwise and any natural weapons the creature also uses are considered secondary natural attacks. These secondary attacks do not interfere with the primary attack as attacking with an off-hand weapon does, but they take the usual -5 penalty (or -2 with the Multiattack feat) for such attacks, even if the natural weapon used is normally the creature’s primary natural weapon.

There you go, thems the rules for that.

Yes, and TWF penalties don't apply to natural attacks.

There's no text that says otherwise, so you treat it as any other natural weapon, as per the rules in the MM. So you can attack with it once as a primary weapon (but only once), or you can add it to your full attack routine with a manufactured weapon as a secondary attack with a -5 attack penalty and 1/2 Str bonus on damage.

The odd thing about the Strength Devotion is it's not clear if the slam is limited to the 1 minute duration, or if you get the slam all the time.

Then there's the "If you already have a natural weapon", which gets really bizarre. Bite, tail slap, tentacle, doesn't matter... but it doesn't say if you get both natural attacks, or your the first natural weapon just gets a +2 damage bonus. Very odd.

Strenght devotion is horribly worded. meh.

By Raw, its an extra attack, since of the . and the "In addition" clearly indicating that its not part of the first statement (bypass all hardness for 1 min).

But yea, that works well, (sneak attack build)
