When you're looking through any book from 3.0 that wasn't updated to 3.5, and it tells you something is a free action, but it feels more like a swift/immediate action, do you ever alter that rule to reflect that change? An example being quicken spell. Used to be a free action only usable once per round, now a swift action. There are other once per round only free actions, so do you normally convert those as well?

Swift actions were introduced after PHB 3.5, which listed Quicken as a free action useable once per round. So this isn't just an edition issue.

There are plenty of up-to-date things that proc as free actions once per round. I've noticed a number of spells that go off with this frequency, most of them 3.5 (Cloud of Knives or Darkbolt for instance).

Generally, make something swift if it's spell-equivalent or can otherwise break the action economy. Otherwise, leave it.

Quicken Spell got errata'd or otherwise corrected, so that doesn't count. Given that that's the case, converting at least some free actions to swift actions sounds about right, and certainly most if not all "as a free action once per round"-style abilities should get that treatment.
