Half-illithid is an add-on template and not a half-template, regardless of what it sounds like. I'm fairly sure Half-golem is the same.

There was Half-Machine template in Dungeon Magazine #91
You may check it on Angelfire (http://www.angelfire.com/realm3/migdalia/Template-Half-Machine.html), but it's not completely correct (Say, magazine version had more bonus feats...)

Where did you get the Heartless/Nobody templates? I'd like to see them.

Heads up: those two templates (and much of the content on D&Dwiki) are homebrew (and D&Dwiki is particularly notorious for being full of bad homebrew).

The pages on that site that are actual official content are clearly labeled "SRD" and are listed here (http://www.dandwiki.com/wiki/SRD:System_Reference_Document). Or, better yet, just use the d20srd (http://www.d20srd.org/), which has all the same information, better presented, without pages and pages of terrible homebrew.

*edit* re: Your original question, nearly every 3.5 sourcebook includes one or more templates; the Monster Manuals of course have lots. Here (http://www.wizards.com/default.asp?x=dnd/lists/monsters&tablefilter=template) is a not-even-close-to-complete list from the Wizards site, and here (http://community.wizards.com/forum/previous-editions-character-optimization/threads/1117261) is a handbook that lists all the author could find.
