Other than crazy cultists and beholders.

I'm working on an Elder Evil campaign based around 4e's Allabar (who's basically atropus if you crossed out undead and wrote far realm in its place) and I can't really think of that many low to mid level enemies to throw at the party and slapping the (CA) pseudonatural template on enemies feels like a copout.

Evil Cultists and their manipulated mooks should be their main antagonists. Far Realms Creatures should be played up.

The Kaorti from the Fiend Folio are humans/beings from the Prime Material plane which were corrupted by Far Realm energies (or something similar). They could make nice Far Realm-themed enemies without necessarily going full C'thulu.

Aren't there Corruption Elementals or whatever from Heroes of Horror that make creatures go evil and crazy by attacking them? Seems like some refluffing could make a madness Elemental.

You could also just use Squid/Octopus from the MM then apply templates to make them useful.
Heck just fiat them a climb/fly speed and they're immediately creepy.

Low CR dudes with the non-epic Pseudonatural template should work too.

Additionally you could use the Sanity rules from Unearthed Arcana. And to make it creepy only use them when the PCs look at/into the eyes of otherwise perfectly normal NPCs. Who of course try to brutally murder the PCs eventually in some ritualistic fashion or another.

Refluff the aforementioned Gibbering Mouther as a humanoid shaped creature wearing a tattered robe and it's a thing of terror.
Same goes for any ooze-ey type creature really.

The Far Realm is a place of inconcievable madness and not-right-no-matter-how-you-look-at-it-ness. Portraying that in game is less about the stats and powers and more about the ambiance. Sideways eyelids, tentacles where they shouldn't be, a reverence for all things insane, physical objects that exude warped space, villanous NPCs who not only cannot die but don't even understand what you mean when you tell them you want them to die as you're driving your sword through their guts. Guts which appear to be made of hands which are in turn made of slugs and worms.

All of which is just descriptive text. Descriptive text that is heavily hinted at and alluded to far in advance so that when it finally comes to fighting your own reflection in a dark alleyway you can't believe you didn't see it coming when your shadow came right out and told you that your reflection was a liar and a traitor. Sure your shadow was a murdering butcher but he was an honest sort. Would never tell you eh wasn't about to cut out your spleen.

TLDR: Portraying the Far Realm, and the Horror genre in general is more about atmosphere and less about mechanics. Conveying that, "The supernatural will get you and there's nothing you can do about it.", is key.

Edit: Also, I've always been a big fan of the Blood Elemental (http://www.wizards.com/dnd/article.asp?x=dnd/sb/sb20030208a) and its "incomprehensible" ability. Very appropriate for creatures that shouldn't be comprehended.

Aye, Gibbers.
I ran a farplane campaign once, I teemed some Gibbers with Advanced Chokers, it went really well.
Also, at higher levels, I ran the farplane was invading, with Rukanyr(FF) as siege engines, and Skybleeders as air assault.


but I have an unnatural love for Rukanyrs.

most any Abberations will work.



ooh! and I had villages of people turn into bloodthirsty ghoul-like Nothics, from Miniatures Handbook. wonderful at low levels.

Sometimes it's just about refluffing the right things.

Example: Planestouched with the equivalent of Aberration Blood feats. (or possibly reskinned raptorans.) Ancient texts refer to them as "The Angels of the Dead Stars." Starspawn would be a nice touch. However, don't describe them as having wings. Describe them as being wreathed in long, cillia-like tentacles that grant flight by reaching out and grasping things that aren't there.

You can also reskin various outsiders for the job. Obyrith demons are the easiest ones, but Nerra, Xerfilstyx devils, and Voor Yugoloths also work nicely.

Tack on the Damtalion Vestige on anything
Other binders have pretty far realmsy stuff
Have a nearly overwhelming encounter, if it goes south (TPK) have them wake up before the encounter... And hint that the same creature is coming for them (they find the same ruins, hear the same snap or scream... Or if not identical, very similar) make them afraid of the battle they premonitioned, and see if they run. If they continue on to battle they ought find something "normal" and "peaceful" instead.
If not quite a TPK but mostly, buff into TPK and finish as above. Regardless they get XP.

Otherwise proceed as normal

Eliminate outsiders if you can, impede planar chitchat. Other realms are quarantining the material plane

Look at the eberron quori
Give them intelligent symbionts as treasure
