I recently got Don't Rest Your Head, and while most of the mechanics make sense to me, I'm having trouble wrapping my head around combat. It seems that Exhaustion and your Flight/Fight responses basically take the place of health/HP, but in fighting a foe, is it just up to the GM when someone is defeated and when they are just hurt? If a PC fights Mother When, does succeeding in one die roll defeat her? That seems improper.

Is there any mechanical difference between fighting and running from someone?

Also, for Madness Talents, I get how you set the number of Madness dice you need to roll depending on the power of the ability. But do you make a separate roll (Discipline + Exhaustion + Madness) to activate your power, or do you use your power during your next action and roll then? To use an example, do you roll to teleport then roll again to attack, or do you roll to teleport in order to get a dive attack on someone?
