I want a way to talk to everything in D&D but cant think of what item does that that is not a custom created one.

A Helm of Telepathy gives a way to communicate to anything with an Intelligence score of 1 or more (27,000 gp).

A Helm of Comprehend Languages and Read Magic (Magic Item Compendium) allows you to understand any language, spoken or written, but doesn't allow you to speak the languages (5,200 gp).

A Crystal Mask of Languages (MIC) grants five languages. Stack those to get access to every language if you really want to (2,500 gp).

A Pearl of Language (MIC) grants one language (600 gp).

Which, really, shouldn't be an epic feat. After all, even a non-optimizer can hit the requisite score easily as well as the skill requirement.

If you OK to start with 6 outsider HD and LA +5, Bralani (http://www.d20srd.org/srd/monsters/bralani.htm) Eladrin or Hound Archon (http://www.d20srd.org/srd/monsters/archon.htm#houndArchon) have always active Tongues as (Su)

Tounges doesn't do reading, you need it and comprehend languages, maybe read magic as well.

In D&D you can, as a baseline, read and write any language you can speak, unless you're illiterate (See 3.5 PHB page 12 under Literacy, for example). The rules on Tongues does not exclude reading and writing. I'd say it's a fair ruling that it lets you read and write things you get to speak, as long as you're not illiterate.

Being able to speak and read all languages in existence, even ones no outsiders have had any contact with, is not an epic achievement?

Not when a level 2/3/4(bard/wiz/cleric) spell can do it.

Not with those prereqs it isn't. Hell, any race with an INT mod of 2 or better can hit it at level 2. A race without a mod can hit it at level 4.
