Looking for a specific Monster

I've tried google fu to find the monster that can cast polymorph any object at will. im pretty sure its a spell like ability but im not completely sure.

Do you remember what creature type it was?

Did it have an artwork?

If it had an artwork what did it look like?

The only ones I can find that weren't errata'd to have a different SLA or Change Shape or something are:
From Book of Exalted Deeds: Morwel, Queen of Stars; Faerinaal, the Queen's Consort; and the Tulani eladrin
and, from Kingdom of Kalamar's 'Dangerous Denizens', under Sprite, the Leprechaun

I don't believe it ever got an official 3.X update, but the Leprechaun in both Tome of Horrors (http://rpg.drivethrustuff.com/product/3019/Tome-of-Horrors-Revised?it=1) and the Creature Catalogue (http://creaturecatalog.enworld.org/cc/converted/view_c.php?CreatureID=1065) has at will PAO.
