Is there a way to get an unarmed strike that can stun, apart from Stunning Fist? Ideally there wouldn't be spells involved, apart from long-term (1/day or less) buffs.

Yes, on criticals
See the Staggering Blow feat from Dragon Compendium

Necklace of Natural Weapons (Savage Species or here (http://archive.wizards.com/default.asp?x=dnd/fc/20060707a)) + Torturous property (+1 enhancement, Ghostwalk). Every hit, Fort save DC 12 or be stunned for 1 round. Increase the enhancement to +2, and the save goes up to DC 17.

As above with the necklace, but use the Sudden Stunning property in MIC.

Thanks! I was worried this would need some sort of spellcasting to work, but it seems like I just needed to look harder.
It's quite a hard one to get to, and obviously requires (Draconic) Polymorphing, but you get a Strength based Stun if you hit with a Spiked Chain.
With 20HD, and +8 Strength from Draconic Polymorph, on top of your +Str Belt, you're dealing with a DC34 Fort save Stun for 1d4 rounds.
Also, you can request a custom magic item which provides the Stunning Fist feat using the A+EG. You can upgrade it with the Extra Stunning (and Ability Focus etc) feat as much as you want. You can pick up such an item through the Ancestral Relic feat.
