Template creation rules!

Well....Self explainatory.
i know i have seen this somewhere and im scrolling along the books trying to find it....with no luck.

Does anybody know where are the RULES FOR CREATING TEMPLATES????

Savage Species has some guidelines for determining level adjustment starting on page 11, but they tend to over-value melee bonuses. Other than that, I can't really think of anything.
