I've seen an epic-level adventure where the players are transported (through one means or another) to another plane of existence. The plane was really interested and original and they basically had to sort through all the issues there.
I think it was for 3.5e but it may have been 2e. I'm VERY uncertain, but I think it was done by Monte Cook but I can't find a complete bibliography of his (although that might just be that there's so much).
