I see no mention of concealment in the rulebook. Have they removed this concept entirely? Is it physical hard cover or nothing?

Cover is brought up in the Basic on page 74. It has references to Concealment at the end.

Concealment is not brought up anywhere else really.

Now, spells like Invisibility do not actual convey Concealment now but a specific status of Invisible.

The term concealment from previous editions has been changed to obscured in this edition for a few cases, and cover for others.

For example: The general rule is that you need be heavily obscured or behind/under full cover to make a hide check. Some abilities (such as a wood elf or an halfling's racial abilities) change the general rule to a specific rule wherein they only need to be lightly obscured under specific circumstances.
