A level 1 monk gets flurry of blows:
-2/-2

Now with 2 weapon fighting:
-2/-2/-2 or is it -4/-4/-4?

The latter. The penalties stack as they come from different sources.

If memory serves me, you should instead have -4/-4 for your main hand and -4/-4 for your offhand (assuming you're dual wielding monk weapons)

Flurry of Blows (Ex)

When unarmored, a monk may strike with a flurry of blows at the expense of accuracy. When doing so, she may make one extra attack in a round at her highest base attack bonus

One, not one per weapon.

Why did I never think to TWF with a monk?! O.O
What is that capped? 10 Attacks/Round? (assuming you're getting off a Flurry) Granted, it would be like 16/16/16/11/11/11/7/7/7/2, But that's pretty beast, especially if you're stats are good enough to help combat the penalty and with Weapon Focus.
And That's without the extra from Snap Kick.

Monks can be made to lay down the smack; they just need a little more work.

You can also layer on rapid shot if you're throwing shuriken:

So -2 flurry, -2 twf, -2 rapid shot for:

-5/-5/-5/-5. Still, it's 4 attacks. (-5 because rapid shot requires point blank shot which nets +1 attack).

-6/-6/-6/-5, actually (or -6/-6/-5/-5 if you're TWFing with the shuriken). Point Blank Shot only applies to attacks made with ranged weapons, but Rapid Shot applies to all attacks made in the round.

All of the shuriken attacks are ranged. Thus, they all get the benefit of Point Blank Shot.
