Death in Ptolus

This is an issue that's been really confusing me. Since the souls seemingly have nowhere to go due to the unique nature of the plane, what happens when someone dies?

Why wouldn't they cross planar boundaries like they do anywhere else?

It's the nature of the setting: Entering the plane is easy, leaving impossible. Summon Monster and similar spells get around this by summoning 'projections' of the creatures instead, but thus far I have found no mention of what happens to the dead.
