I read something about that in the Grand History of Eberron, but I haven't seen it anywhere else; I know it is known as extint, but it really is?

Well, the grand matriarch leader of the Blood of Vol cult does have it, because she's of course Lady Vol, the founder of the entire religion dedicated to herself, and the whole reason for why everyone tried to destroy the mark of death, but that's not really common knowledge.

For all intention and purpose, the dragonmark of death is as good as extinct, and there's technically no way for Vol to pass it on, because she's now a lich.

As a GM, you could of course say that that's not true, and that Vol's not the only one to have it, or that Vol sired some kids before turning herself into a lich, who are now passing the mark on, or some random planar shenanigan causes that dragonmark to appear on another humanoid race.

I'm using the restoration of the Mark of Death as Vol's primary motivation. Possibly time travel involved.

The Mark of Death (http://eberron.wikia.com/wiki/Mark_of_Death) is one of those areas of the setting left deliberately as a blank canvas, much like "What really caused the Mourning?"

Basically the mark is as extinct as the DM wants it to be.

Another good use for the BoEF. Nothing a little polymorph can't fix. Just find a nice cooperative lady and let it happen.

Nope, just a few suggestions on one of Baker's blogs. It would have abilities about death, not causing death. Ie suggested effects were things like Speak with Dead, Gentle Repose, etc. Actually killing people would be more of an Aberrant Mark thing.

Well, you could probably start by killing her followers, killing her, tracking down her petitioner... so on so forth.
