A Fighter 5 takes the Attack action, attacks once, attacks with her off-hand weapon from TWF, then attacks again with her main hand. Then, if she uses her Action Surge for an extra action, she can do it all again for a total of six.

Is this correct? If so, that could be rather powerful, especially with the Dual Wielder feat. What do you think?

Nope action surge gives another action but not a new bonus action so will be 5 attacks.

No. Action surge gives an additional action, but the the PC's turn is still limited to one bonus action. A 5th level fighter using TWF style would get 5 attacks. 2 for the first action, 2 for the second action, and 1 for the bonus action.

That's why TWF can fall behind in damage and isn't recommended for fighters. They have enough extra attacks in their attack action that the larger weapons do more damage, and can get that bonus action regardless as a shield master, polearm master, or high likelihood with great weapon master. EDIT: I should say TWF is possible in a build, but normally not optimal without the magical weapons to make it worth it.

Action surge gives an additional action not an extra turn so you do not get an extra bonus action. This means your total attacks are 5 (4 main 1 off hand).

Aah, okay. Thank you all for that explanation!
