I have a friend who wants to run an s-1 Tomb of horrors, updated to 5th ed.
I plan to make a shifter druid to play in it, we will be 12th level. I'm looking for advice on forms ECL 2-4.

Forms come in CR, here is a link to some druid guides.

You'll have to find a homebrewed shifter; there is no shifter in 5e presently.

Unless 'shifter' refers to Moon druid, of course.
@ OP: I have a fairly thorough overview of beasts for shapes and summoning in the second post of my guide. Link in my signature.

I was run through this module all by myself playing Merlin straight out of the Deities and Demigods, 2nd edition. I used Polymorph Other to turn myself into a succubus and had succubus powers all through it, including being able to "Shapechange" into any humanoid. Merlin is a high level druid as well as a magic-user (they weren't called wizards for some reason). I made it all the way to the last encounter and then died. It was fun.
Be super paranoid. It's justified.
