Blackthorn Lycanthrope

So there is a lycanthrope template in Dungeon Magazine, and I have the issue on my hard drive... which is currently three states away and I need the template. So, does anyone know which issue The Blackthorn Lycanthrope template is from? I kinda need it for todays game and google keeps sending me to fanfictions.

Actually, it's Blacktooth Lycanthropy, and it's in the Dungeon Magazine #129
