How would you resolve grabbing, picking up and throwing an opponent?

I'm thinking you'd have to grapple them normally to start with, then next turn you'd have to make a grapple check, and if that was successful you'd throw them as an improvised throwing weapon with a 5ft. range increment, dealing 1d6+Str (assuming a Medium enemy) to both enemies. Throwing would take a full-round action because of how bulky people are compared to, say, a javelin.

You'd have to be able to lift them, of course, but should it be capped at a medium or a heavy load (light is too little to lift anyone)? And should the thrown and struck opponents have to make a check or a save to avoid falling prone?

Or if you want to do it the easy way, you can be three size categories larger than them and use the Snatch (http://www.d20srd.org/srd/monsterFeats.htm#snatch) feat.

The Bloodstorm Blade gains the Throw Anything feat, which is essentially Ranged Exotic Weapon Proficiency [Insert Living/Nonliving Object Here].

I'm working on a build that does exactly that, actually (more in another topic).
I'm using the Hulking Hurler (CW) to throw them and the Justiciar (also CW) to tie them up, first. Their Hog-Tie and Improved Hog-Tie abilities allow them to tie up or manacle-ize their opponents once they have them pinned and grappled, respectively.

One of the schools in Tome of Battle has a whole set of maneuvers to hurling people around.

Yeah. The Setting Sun Discipline delineates a throw as a trip, no more, no less. Particular throws (such as Ballista Throw or Devatstating Throw) require other rolls, but the basic throw, Mighty Throw, is merely a trip check that throws your opponent 10'.

Yes, Setting Sun. There is no normal way to throw someone you're grappling, but you could Bull Rush them.

There is a feat in Races of Stone that lets you do this; however you have to be large larger.
