Well, here goes, I guess: I accidently hit enter while trying to type the title, and before I knew it, it had posted before i was ready. There we go. I'd say something about feeling stupid, but then again, I think the monster can speak for itself.

Anyway, this should be a testament to why I should never be allowed to DM, not the least of which would be finding reasons to use the d5 during play. So without further ado, I present:

The Surprise Elemental

Of all the elementals adventurers least expect to face, the elementals of surprise are likely not on the list at all. No one knows where exactly these creatures come from, but one thing is for sure: when they appear, confusion soon follows.

A surprise elemental can appear as any number of things, from an swirling amorphous cloud, to an ordinary human, to a giant green flying earthworm. It's form can shift at any time for no apparent reason, but it generally stays within the given size of the elemental.

Surprise elementals gain a +8 racial bonus to Hide and Move Silently checks.

Surprise elementals attack in no discernable pattern, jumping randomly between opponents and switching targets for no reason. Generally they will find a way to attack someone who thinks he's safe from assault.

Special abilities:

Variable armor: Roll for a given elemental's natural armor bonus every round, or just once for the encounter, whichever you prefer.

Random attack: Each round, the surprise elemental attacks with a random natural or weapon attack. The natural attacks may be of any normal variety, such as slam, bite, claw, gore, wing buffet, etc and the weapon attacks may be of an appropriate sized weapon that materializes, which the surprise elemental is considered proficient in.
Weapons dematerialize at the beginning of the elemental's next turn, unless it randomly uses the same weapon twice. The elemental is not proficient in any weapons otherwise, so even if the elemental used a longsword as it's last random attack, it won't be proficient in a longsword it picks up off the ground.

Element of Surprise: A surprise elemental always gets a surprise round to act in at the beginning of combat, regardless of circumstances. Other creatures may not act in this round unless they make a Will save at a DC equal (10 + 1/2 elemental's HD + Cha mod) to be able to act in the surprise round. Other surprise elementals may act in this round as well.

Surprise Attack: A creature attacked by a surprise elemental must make a Will save (DC 10 + 1/2 HD + Cha mod) or be flat-footed against that attack. A creature must make a save for each individual attack the elemental makes.

Sudden Strike: A surprise elemental has the Sudden Strike ability, with bonus damage dice as a ninja of the same level as the elemental's hit dice.

Dimension Door (Su): A surprise elemental can teleport as per dimension door (caster level equal to 8, or HD, whichever is greater) as a free action once per round and can act immediately after teleporting.

Random Action: Every 1d4 rounds, a surprise elemental acts randomly. Roll on the chart below:
01-20 Put on a performance or make a simple item (based on the given skill the elemental has)
21-25 Flee from combat. If this is rolled, the elemental doesn't return. Not as long as it's expected, anyway.
26-50 Attack nearest creature
51-65 Act Normally
66-95 Gain the special ability of a different elemental type for one round and use it. Lose Sudden Strike and Surprise Attack for the round.
96-00 DM's option (be creative)
