I could have sworn I've seen a flaw which causes you to always count as flanked when threatened. Am I just hallucinating it?

Unless there WotC published any flaws besides the ones in Unearthed Arcana I'm pretty sure you're imagining things, or are thinking of some homebrew you stumbled across.
