Came accross this fantastic piece of art by Howard Swindell today:

...I was wondering how I could use it as a D&D monster?

My first thought is a Gravetouched Ghoul Multiheaded Hadrimoi (MM3 p.185).

Any other takers?

Hmm, I see an Aberrant Limbs, Gravetouched ghoul, Ettin with an undead grafted arm.

Looking at the MM, the Ettin is a little different looking, but if you go off how the mini looks, which I am because that is how I think they look, then that pic looks pretty close to what I imagine my creation would.

For a medium version, just do Parasitic Twin, with three undead grafted arms (Enervating Arm, Paralyzing Arm, Weakening Arm) and you choice of gross undead template.

well with what I think is five arms maybe an Akutsukai/Akutenshi (OA) made from a multiheaded human even ignoring the taint part of them its a nasty chasse to build something off of. Or maybe something using the symbiote template you could work something out...

You know, this seems like a good idea for a permanent thread. Matching monsters up to pictures.

With that said, what monster would be a good fit for this?

Oh geez, giant dude with a portal inside him...

Maybe a Fiendwurm (MM2), changed to a Gargantuan (Tall) Outsider with a different destination plane (and different subtypes appropriately)?

Also, I definitely approve of this being a permanent thread. Someone posts a picture, the rest of us try to recreate it sticking as close to first-party content as possible.

As for the portal-giant, I would likely go with a construct of some sort but have no specific suggestion for the time being.

Well, book of erotic fantasy is not first party, so I don't think we can adequately get there.

And portal Dude looks Kinda like a sleeker Ptah from Deities and Demigods 3.0.

I can't find him, but I was thinking of that immortal, colossal, fleshy creature that was all "Mouth" themed. If it eats you, you get sent you the layer of the Abyss it reforms on when slain, which I think is still mouth-themed.

Dalmosh, from MMV (pp. 16-17). Hmm, I wonder if one could make a construct with it as the base creature...
