Just string together a bunch of dungeon delves. Don't bother to come up with even the barest thread to connect them narratively.
If you build it, will they come?

I was never a fan. I know it was infamous, but was it actually popular, like people played it?
I ran an Undermountain game for a few sessions, but it fell apart for other RL reasons. Never got to see if it would have long-term appeal.

That's how a lot of classic campaign worlds started. Dungeon with a town on top.
If you have a large and inconsistent group, it can work very well. Typically, after a while subgroups will start doing more "plot-like" stuff, but with the dungeon always there to entertain people, the game becomes very resilient to player churn.

Make it good enough and nobody's going to miss meeting up in a tavern.
