(Possibly) in before the deluge of comments ... Paladin and Monk are typically considered very weak classes. There are other ways to get the same sort of theme without using those two classes specifically, depending on what sources you have available. Cleric to Prestige Paladin is a thing, and it's in the Open Game License part of Unearthed Arcana; Sacred Fist from Complete Divine is a class that combines a Divine caster with Monk. (There is a well-known inconsistency between the table and the description; check with your DM to see how many caster levels they think the PrC will give you).

If you're certain you want to proceed with Paladin and Monk, the Ascetic Knight feat from Complete Adventurer lets your Paladin and Monk levels stack for Smite and Unarmed damage.

Telonius covered pretty much all of the major bases. There are a few other ways, but those are the big ones.

Another option is the Fist of Raziel class from Book of Exalted Deeds. This is a great class generally, and a very smite-happy one at that. It's not build around Monks, but around martial smiting; that said, because of the extra smite power it offers, it can make a Monk into a very smite-happy unarmed combatant.

If you're running PF, there's the Champion of Irori (http://www.d20pfsrd.com/classes/prestige-classes/other-paizo/c-d/champion-of-the-enlightened). PF Paladins and Monks tend to be slightly more potent than their 3.5 brethren, and this class synthesizes them well, basically turning the character into an unarmed smiting machine.

The Argent Fist, from Faiths of Eberron. It advances smite damage and unarmed strike damage, and effectively grants the Ascetic Knight feat at first level. If you already have the Ascetic Knight feat, you even get to pick a replacement feat, which is quite thoughtful of the developer. You also get to keep wis to AC even when heavily armoured, you get mettle, you get bonuses on stunning fist attacks that are also smites, and you get some extra uses for turn attempts. Standard entry: monk 2/paladin 4.

Pick up Serenity (Dragon Magazine 306 feat) to switch your Divine Grace from cha to wis.

There is also the enlightened paladin archetype, which is pretty much a Paladin/Monk PRC without the PRC part.

Crusader / Shadow Sun Ninja

Take Martial Study twice to open access to Setting Sun and Shadow Hand disciplines.

Once you have a manoeuvre from a disciple you can add more.

If Pathfinder stuff is allowed the Champion of Irori Prestige Class rocks(as does the Pathfinder Paladin)
