So, the question is simple:

How and where would you build a prison that is inescapable?


Sam's Alcatraz:
The prison is at 5 miles in the sky. It has walls that are 30 cm thick and made of Adamantine. Everywhere are Shield Guardians and the Courtyard is in the water with water weirds as guardians.

If you want an interesting look at an inescapable prison, I'd advise looking up the Birdcage, from Worm.

An antimagic field is centered around you. And for example a Monk could just bang until the walls break.

Two large adamantine domes with tubes connecting them, filled with grates of adamantine as well. One tube has gates it it which can only be opened from the side facing the right dome. The left dome is kept constantly under the effects of antimagic fields to prevent magic users using teleport to break people out. The other is kept constantly under the effects of multiple daylight spells, and is kept full of young trees, vines and other plants, which are felled and removed whenever they grow old. In the centre of this forested dome is a guard tower, where those who run the prison sleep. They have a magic circle for movement of prisoners and supplies in and out. The entire structure was built without a physical way in or out of the domes, not even for air, then moved out to sea and dropped into the ocean. Then was buried in sand, rocks, and finally huge amounts of barrels of fertiliser, chum and the like, so as the barrels degraded they'd lure a large and deadly ecosystem to form on top.

Here is two ideas:

1. What about a pocket dimension? Wizard created, only a dimensional gate to get there that is activated with a unique ritual. The prime material gate location is in a hard to reach place, either a floating moving "island" or in a volcano.

2. Or you could go with something I remember from an old AD&D module "The Forgotten Temple of Tharizdun"
In that module Tharizdun was sill trapped and sleeping, reachable via a special ritual which opened a magical stairway deep in the earth. This stairway is solid rock unless the Ritual is performed....Pretty cool set-up.... needed to blow a special horn, while wearing specific robes in a room that was extremley cold (the robes protected you).The prisoners are all "sleeping" in cocoons.

First, you have to ask yourself the question: 'why is this prison here?' What is powerful enough to need such a giant building just to keep it detained? Why can't people just kill it and be done with it?

As for the prison itself, how about Flesh To Stoning everyone inside, then hiding them amongst hundreds of other, similar statues? It'll ensure the prisoners can't escape or die of natural causes while still leaving them 'alive', as well as making someone's job of freeing them much harder.

Gating in some outsiders might be a good idea. Yugoloths would probably agree to guard something in exchange for a lot of gold. Then again, they are easily bribed, so they may not be the best of choices.

I recommend undead or constructs for prison guards. They don't need food, water, or air (allowing for airless areas the prisoners won't be able to traverse), don't have to sleep, can't easily be bribed, and will follow your orders no matter what.

Killing an extremely powerful being will just release the soul to be True Resurrected by its cleric allies.

True, and some creatures will be worse when they go to the Lower Planes if they die. And on Earth criminals dont always get the Death penalty right? Some criminals get locked up for the rest of their lives. Also torture is something Archmages like.
