Was there ever a feat or magic item or anything in 3.5 or Pathfinder that let you make ranged attacks in melee without provoking opportunity attacks? Or at least gave a big bonus to AC against the OA?

I could've sworn I'd seen something somewhere...

I think you're looking for Point Blank Master (http://www.d20pfsrd.com/feats/combat-feats/point-blank-master-combat), available only to Fighters, Rangers, Zen Archers, and things that get Effective Fighter Levels (so like, half of all melee focused archetypes.) I exaggerate, but not by much

Dream Scarred Press's Hawkguard (http://www.d20pfsrd.com/path-of-war/classes/warder/warder-archetypes/hawkguard-warder-archetype) also gets non-provoking fire/reloads on a ranged weapon of choice at 3.

2 levels in Order of the Bow Initiate (complete warrior) does the trick (close combat shot). ctrl + f on 'close combat shot' in the list of stuff (http://www.minmaxboards.com/index.php?PHPSESSID=3ilkpu1mbs78lt65hjk29q7ra2&topic=1412.0) gives more results.

Any item that lets you cast the Arrow Mind spell—a wand, for instance—will do the trick, at least for bows, anyway.

There are class features, like the Order of the Bow Initiate's Close Combat Shot and the Master Thrower's Defensive Throw (not to be confused with the feat of the same name).

For Feats, there's No Threat To Me (Dragon 355 p90) which allows a character to ignore a (type of) Favored Enemy when making ranged attacks.

I've seen plenty of home-brewed feats that do that, though.

There's also Illusion of Calm (http://www.d20pfsrd.com/magic/all-spells/i/illusion-of-calm), though that only protects you if you're using thrown weapons or firing ranged spells.
