A plague golem is a monstrous creation made from rotten and cancerous flesh and various diseased fluids. A shambling inhuman mass filled with acidic pus and diseased bile, a plague golem can be made for many different reasons, but all truly have one purpose. Originally designed by arcanaloth servants of the Oinoloth instructions for their design was spread throughout the Prime Material Plane. These constructs are animated not by elemental spirits, but by a fragment of the Lower Planes, filling them with Evil energies and making their creation an act which taints the soul of the maker. Due to this other fiends have taken to spreading their design, creating golem manuals which aid in their construction.
A plague golem’s appearance is repulsive and its odor repugnant. Made from diseased flesh a plague golem is a mass of sores, cancerous tumors, leprosy, and gangrene. It shambles on over thick legs, its body jiggling with the diseased fluids which fill the outer flesh. Its proportions are a mockery of humanity, impossibly broad and fat, its neck non-existence, its face flat, no ears on the bulge of a head. Its hands bear two or three fat fingers, its feet strangely rounded like an elephants. A plague golem is 9’ tall but weighs over seven hundred pounds.
A plague golem is often created because they are intelligent constructs capable of following more complicated instructions. A plague golem will carry out instructions obediently and efficiently, but they have desires beyond obedience to their creator. A plague golem has a powerful desire to spread disease. If not ordered to kill it will usually allow a diseased opponent to flee, and it will look for any excuse to spread disease while fulfilling orders.
Combat:
A plague golem begins by approaching a foe with its disgusting odor which fills living creatures with nausea. While they are retching it approaches and begins to pummel them with its fists inflicting a variety of random diseases. If it cannot close with a creature for some reason it will instead use its acid spit to strike at them from a distance. A plague golem avoids using its acid spit when it would provoke an attack of opportunity to do so. Despite their intelligence a plague golem has no fear of destruction; they will continue to fight as long as it is in line with their orders even if it means their death.
A free willed plague golem is more likely to retreat but even they rarely do so, preferring instead to beat a foe into retreat or die trying.
Acid Spit (Su): A plague golem can spit a blob of acidic pus and vomit out to a range of 60 ft as a natural weapon and may use it as a primary natural weapon. This is a ranged touch dealing 2d6 acid damage and inflicting disease.
Acid Spray (Su): A plague golem can release a spray of acidic, excrement filled vomit in a 30 ft cone once per minute. Any creature in this cone takes 8d8 acid damage with a Reflex save (DC 17; Con based) halves this damage.
Berserk (Ex): When a plague golem enters combat, there is a cumulative 1% chance each round that its Lower Planar spirit breaks free and the golem goes berserk. The uncontrolled golem goes on a rampage, attacking the nearest living creature, then moving on to spread the plague. The golem’s creator, if within 60 feet, can try to regain control by speaking firmly and persuasively to the golem, which requires a DC 21 Charisma check. It takes 1 minute of inactivity by the golem to reset the golem’s berserk chance to 0%.
Each day a plague golem is not given the opportunity to spread plague there is a cumulative 2% chance that it will go berserk in a similar manner.
Disease (Su): A plague golem is a walking mass of disease and plague. Any creature damaged by its acid spray, acid spit, or natural weapons must make a Fortitude save (DC 19; Con based with a racial +2 to the DC) or immediately suffer a random disease from the table below. This disease has an instantaneous incubation time. If the target already is suffering the disease but it is still incubating it flares up immediately. If the target is currently suffering the disease this has no effect. Subsequent saves against the disease are made at its normal DC.
RollDiseaseDamage
1 Blinding Fever 1d4 Str (on 2+ DC 16 Fort save or permanent blindness)
2Cackle Fever1d6 Wis
3Demon Fever1d6 Con (second save DC 18 or 1 point is drain)
4Filth Fever1d3 Con and 1d3 Dex
5Mindfire1d4 Int
6Red Ache1d6 Str
7Shakes1d8 Dex
8Slimy Doom1d4 Con (second save DC 14 or 1 point is drain)

Immunity to Magic (Ex): A plague golem is immune to any spell or spell-like ability that allows spell resistance. In addition, certain spells and effects function differently against the creature, as noted below.
A magical attack that deals fire damage disables a plague golem’s fast healing for 1 round +1 round per spell level.
A magical attack which deals acid damage heals a plague golem ½ the damage that would have been dealt and reduces the amount of time till it can use its acid spray again by 1 round per 5 points of damage healed.
A Remove Disease or Heal spell cast on a plague golem deals 1d10 damage per caster level to it and slows it for 1d4+1 rounds; a Fort save halves this damage.
A Purify Food and Water spell cast on a plague golem deals 1d6 damage per caster level; Fort halves.
A Contagion spell heals a plague golem 10 hp per caster level of the spell, allows them to use their acid spray again as if a minute had passed, and breaks any slow effect affecting it.
Stench (Ex): A plague golem bears a horrible stench and odor of disease. Living creatures within 30 ft of a plague golem must make a Fortitude save (DC 15, Con based) or be nauseated for 1d3+1 rounds. A failure grants the target immunity for 1 minute before needing to save again, a success grants immunity for 24 hours.
Construction
A plague golem is constructed from a mass of diseased flesh of humanoid creatures sewn together and filled with diseased liquids and semi-solid waste. It is a bag of pox and disease ridden flesh and tumors filled with the sweat, vomit, puss, blood, and excrement of the sick. Parts of the golem must come from at least 13 individuals. There is no necessary cost for the golem, though to get the requisite materials often requires finding a plague ridden community and harvesting its fresh dead, or creating one.
Assembling the body requires a DC 13 Heal check.
