Xykon was probably using his last 5th level slot to try to Teleport Redcloak, and by Redcloak I mean the Phylactery, somewhere safe. Like back with the hobgoblins.

I think he's referring to the fizzle when Soon uses the last Smite Evil. Teleport would be a good choice there, except that I'm not sure Xykon has it. Whatever it was, though, it doesn't much matter, since it did get fizzled (illustrating why caster-types shouldn't try to go toe to toe with melee characters).

When? Where?

The fizzles? Probably another magic missiles or the strongest other stuff he is left. But does it matter? We cannot decide and cannot know it anyway... the important thing is Soon (if you mean the fizzles) breaks his spells with his hits...
