What are the better fears for boosting sneak attack or allowing sneak attack to like always hit?

Penetrating Strike ACF from Castle Ravenloft is a good one (I've been told that the 'Lightbringer' Penetrating Strike is better, but I haven't tried it, so someone else might be better placed to explain that one.)
Also, if you dip into Swordsage you can get the stance 'Island of blades' for easier flanking. (along with some useful manoeuvres)
Also Craven is always a good feat, extra sneak attack damage equal to character level. These are all useful when combined with Staggering Strike (Stagger enemy on successful sneak attack, DC=damage dealt)

1) You want to deny dex bonus so you can use ranged attacks, because then you get to full attack. In 3.5 the best way to do this is a Ring of Blinking, though other options exist, such as grease and glitterdust and anything that stuns people. In Pathfinder you want to dip oracle for the ability to see through smoke, and then run around with an eversmoking bottle, so that you can see everyone and no one can see you.

At lower levels, if you don't have a friendly wizard throwing around status effects that allow you to blow people up (say because your DM is a fool and thinks Rogues would rather have a fighter than a Wizard in their party so he bans Wizards and Sorcerers) you can approximate will all sorts of cheap tricks, like having an intelligent item or item familiar wand of Grease (or some sort of ice floor spell, there's a particularly good Cleric one), and then your item can cast itself and then you can full attack the flat footed enemies.

2) You want to maximize your number of attacks. So Splitting Bows in 3.5, Rapid Shoot, TWFing, Haste, Ect. If you choose to throw things, you can use Rapid Shot and TWFing. If you choose to throw things that are touch attacks, like acid flasks, you almost always hit.

3) Wands of Gravestrike and Golemstrike and Plantstrike allow you to SA things you otherwise wouldn't. Crippling Strike plus Saavy Rogue (from Complete Scoundrel I believe) allows you to do Str damage on attacks even if the target is immune to SA.

4) If you are going to fight in melee, there is a weapon enchantment Deadly Precision from Complete Adventurer that lets you do +2d6 damage, this damage is not actually SA, but triggers whenever you would SA, since you are already Blinking or whatever, it basically means that you do an extra 2d6 damage on every attack, even if the enemy is immune to SA.

5) Get more SA damage. Usual things here are Rogues Vest, and you can either spend feats, or a one level Swordsage dip for 2d6 from Assassin's Stance. If you already have Crippling strike, that is always worth it, if you don't already have it, if you were never going to get high enough level to get it because the game will end before then, it's totally worth it. Craven also goes here. Also Halfling Rogue substitution level in Races of the Wild that gives you 2d6 Ranged SA at level 1, so as a level 6 Halfling thrower you can be looking at 6d6+6 Sneak Attack added onto all your attacks when the enemy is glitterdusted or greased.
