Someone uses Wood Shape or Stone Shape to mold restraints onto a helpless creature. They might just make really snug and solid manacles, or might go so far as to encase whole limbs into a tree/wall.

How should one go about determining Break and Escape Artist DCs?

I'd use the DCs listed here (http://www.d20srd.org/srd/exploration.htm#breakingItems) as a base and modify them depending on just how much restraining is going on. For example, I'd put wood/stone manacles at DCs of 21 and 24, respectively, and use normal manacle Escape Artist DCs (http://www.d20srd.org/srd/skills/escapeArtist.htm). If you were to encase entire limbs, I'd increase the DC by +2 for each full limb encased, but require a nearly impossible Escape Artist check (http://www.d20srd.org/srd/epic/skills.htm#escapeArtist) if it was a full body encasement.

Break DC seems simple - just use the break DC of the material they're trapped in.

Escape Artist lists 35 as the DC to escape masterwork manacles, which is what "really snug and solid" manacles would be. If the creature is encased in stone or wood entirely, the DC for tight spaces (30) seems appropriate.
