I've been looking around at various tips for this subject, but I haven't found anything regarding formatting and how much/little detail you should include. I want to make something similar in size and depth to Princes of the Apocalypse and the Starter Set (Lost Mine of Phandelver).
I'm working on drawing up maps and towns for this, as well as including some of the homebrew I've made as well as some from around the Playground/Middle Finger of Vecna/ENWorld/etc.

I know you need:
-Title page
-WotC copyright disclaimer sheet (if sharing it)
-Table of Contents
-World Lore (Style (High magic, low magic, steampunk, whatever), History, Geography, important NPCs)
-Race/Class additions
-Chapters Sorted by place/encounter/theme
-Monsters, Items, Spells in an Appendix
-Index
-Handouts/Maps

Is there a guideline for how everything should be worded, or a free WotC example of a 5e adventure I could use as reference? And should I break this into several different adventures format-wise by level tiers? Or just include 1-20 in one campaign book?

I would Highly suggest checking out the DM's Guild (http://www.dmsguild.com/) They have templates and guidelines readily available.
