For the low hanging fruit, get a wizard to PaO you into the highest form possible, get level drained to one and use a thought bottle to relevel with your now very high intelligence score.

Somewhat of a petty nitpick, but the answer to max skill ranks is 23.

Venerable (from level 1) Human with the deep template and the rogue class is probably the best you get. Nymph's kiss is a must.
8SP/level is the best you get outside of changeling sub levels but the human bonus skillpoint makes up for that fast (it is a total of +8 skillpoints totaI over the 8 levels of changeling whereas human gets +11 points over those same 8 levels

get inherent bonuses to INT asap

racewise: Human, the skillpoint per level can't be beat at la +0; at best you get +2 int which is the same; and the bonus feat works to get...

feat: nymph's kiss; you want it at LVL 1 and you NEVER EVER want to lose it by being evil
you also want the item familiar feat and pour all your skillpoints into it; you probably want a graft or prosthesis so as not to ever lose it

class: rogue or other 8+ sp per level class
templatewise
deep + (up to +2 LA of inherited templates that boost int)
necropolitan ASAP (for fluff... cause u venerable)
acquired templates: get only after you've bought off the LA equivalent to them; ie: never have more than +3 LA

*if you ignore the blatantly obvious typo: your first level NEEDS to be swordsage

Doesn't PaO lower your Int if the form has less than you do, but leave it if it has more? I.e. this doesn't work like that?

no, it sets your int to that of the new form regardless of what it was prior.

You need some more groundrules/limits otherwise:
Step 1: Get 19 HD
Step 2: Get NI Int
Step 3: 1 level of Factotum
Result: All skills at max ranks and still have NI skill points left to spend

I'll take a swing at a few of the easy things.

Human has +24 skill points
18 int with int bonuses at every 4 levels.
20 levels of rogue

level 1: (8+4)x4+4=52
Level 7: (8+4)x6+6=78
Level 15: (8+5)x8+8=112
Level 20: (8+6)x5+5=75
