Computer Trying to remember an old computer game [Archive]

I reminisced about old games I used to play in another thread and I cannot remember the name of this one.

What I do remember:
It had you change into various monsters if you had a enough of a certain resource (you unlocked other forms somehow later on). It had a screen layout somewhat like doom 1 (so like an old fps game) and health potions on the ground. It had a weapon you could throw and it returns to you and a trident as another weapon. I think the first form was a werewolf. No it's not altered beast. The storyline had something to do with you being born with the power to shapeshift.

Has anyone heard of this game?
