I love the flavor, and it's quite distinct from the Ghost in the Machine pact from the Modern Magic UA.

The abilities seem generally balanced and are very flavorful.

My only nitpick is that "you can use your reaction to curse a creature" should be that creature, not a creature.

Before I start off I think I should put down there is Modern Magic's Ghost in the Machine which is in flavor quite similar but you went with a different approach in mechanics. http://media.wizards.com/2015/downloads/dnd/UA_ModernMagic.pdf
1. Spells, a good amount of utility and defensive buffs, overall very fitting. You do have a couple of commas that should be in other places though.
2. 1st level, so if successful you are roughly use an action to deplete them of maybe two actions or one and loss of options. It is modest at best and I think this is touching more on specific setting issues rather than the patron itself.
3. The 6th level is way out of balance. There is no spell, feature, or even condition out there that covers a blanket disadvantage on all saves on top of an unavoidable poisoned condition. You then even tack on half damage for the initial hit. This far out classes any other 6th level Warlock abilities.
4. So, banishment with a game altering rider. At RAW even when it returns the universe doesn't reset and even if you changed it the time being could allow players to ignore the encounter entirely with no difficulty. Ultimately it relies upon the rationalization of the DM, but the amount of factors especially the importance of the NPC in question could be very troublesome to figure out at a moment's notice, even if detrimental.
5. Interesting, not broken in its action at all since it is just two turns plus reactions once a day but broken in just creating a new you. What if the original you was at 1 hit point and had some crippling conditions that otherwise didn't interfere with using this? A complete healthy you is conjured out of thin air for no cost. This effectively doubles your hit points and offers a get out of jail free card to most things. It also doesn't specify where you summon it.
