Could a level 9 pitborn tiefling vampire lunar oracle pull this off while his grizzly bear companion did a bite/claw/claw from behind?

It seems doable, since I'm doing a chest bump attack, then a mouth attack, then two arm attacks, then a horn attack.

The default is "one Natural per 'limb'", and you're mostly good. The only potential conflict is the Slam/Claws.
I can't find a place that flat out says "Slam = Hand/Arm", but there are a couple place that at least imply it.

Natural Attack Rules: (http://www.d20pfsrd.com/bestiary/rules-for-monsters/universal-monster-rules#TOC-Table:-Natural-Attacks-by-Size)
"Creatures with natural attacks and attacks made with weapons can use both as part of a full attack action (although often a creature must forgo one natural attack for each weapon clutched in that limb, be it a claw, tentacle, or slam)."

Eidolon Slam Attack: (http://www.d20pfsrd.com/classes/base-classes/summoner/eidolons)
"This evolution can be selected more than once, but the eidolon must possess an equal number of the limbs evolution."

If I did improved natural attack 4 times, would it be worth it?

Probably only worth it on the Claws.
For the rest it'd be 1 feat for +1 or +2 damage each, which isn't the best use of a feat. Claws at least have the bonus of there being 2 of them to gain from it.
