Is it possible to make a half minotaur half fiend human? How and where is the source/sources

yes, there is. Half-minotaur template is from Dragon 313, and

"Half-minotaur" is an inherited template that can be added to any creature of the giant, humanoid, or monstrous humanoid type (referred to hereafter as the "base creature") from Small to Huge in size.
Humans are Medium Humanoids, so they qualify. The template,in turn, makes you a Large Humanoid.

Half fiend is from the Mosnter manual, and can be found on the SRD.

"Half-fiend" is an inherited template that can be added to any living, corporeal creature with an Intelligence score of 4 or more and nongood alignment (referred to hereafter as the base creature).
So, assuming they aren't good (and have poor stat rolls), the half-minotaur human still qualifies, as they're living, corporeal creatures.

Yep, say for example if a half fiend minotaur sleeps with a particularly adventurous lady (or man).

As for the rules

A half-minotaur is a valid base for the half fiend template. So long as both are applied at character birth (being inherited templates). You can have multiple templates so long as you are a valid target for each to be applied in order.

Another idea, quite similar in both fluff and crunch, is to make a half-goristro (http://archive.wizards.com/default.asp?x=dnd/we/20060630x&page=1). It gives a size increase with all relevant ability changes, as well as natural bonuses to stats and some SLA's.

Nar Fiendbond spell (Lost Empires of Faerūn) can give Half-Fiend template to Half-Miniraur Human (assuming the Human is smarter than Int 3)
