I've actually constructed these templates for a Final Fantasy D20 game that an ad will be up for here fairly quickly.


Bahamut
Natural Armor: Increase Natural Armor by +4.

Senses: Gain darkvision out to 120 ft.

Speed: Grow wings and gain a flight speed of 50 ft with good maneuverability.

Special Attacks:
Blast- By spending 1 MP (non-caster classes are considered to have MP equal to their character level for the purposes of this ability), you may attempt a ranged blast attack. It has a max range of 40 ft and deals 1d10+character level in non-elemental damage. You may spend an additional MP to either have it target touch AC or be used as a Sunder attempt.

Natural Attacks- Gain two claw attacks that deal 1d6 damage each and a single bite attack that deals 1d12.

Traits:
First Among Eidolons- You become immune to fear and charm effects. Add your charisma modifier to all saves against Fiends (Undead, Aberrations, Monstrous Humanoids, Outsiders, Dragons, or Outsiders).
Flare- By spending a swift action you may change all damage you deal during this turn from spells and attacks, into non-elemental damage.

Ability Score Increase: +4 Strength, +4 Constitution, +4 Charisma, +4 Wisdom, +2 Intelligence

Physical Changes: Increase in height, growing wings and claws, eyes change to draconic, skin changes to blue or purple.


Carbuncle
Deflection Bonus: Gain a Deflection Bonus to your AC equal to your Wisdom modifier.

Senses: Gain 60ft of darkvision and Lifesense.

Special Attacks: You may channel positive energy as a Cleric of your character level -2.

Traits:
Reflect- Three times per day you may Reflect a single spell as an immediate action, as per the Reflect spell.
Extend Defenses- Once per day you may add your defensive traits (your Deflection Bonus and Reflect ability) to all allies within 20 ft as a standard action. This lasts for 3 rounds and does not increase the number of Reflect attempts you get per day, but you still choose when to activate it.

Ability Score Increase: +6 Wisdom, +6 Dexterity, +4 Charisma, +2 Constitution

Physical Changes: Decrease in size, growing a single red ruby on your hand or forehead, brightly colored hair.


Diablos
Natural Armor: Gain Natural Armor +2.

Damage Reduction: Gain DR 3/Adamantine.

Senses: Gain 60ft of darkvision. This penetrates areas of Deeper Darkness.

Speed: You may teleport 20 ft as a move action.

Special Attacks:
Dark Claw- Gain a single claw attack that deals 2d6 slashing damage + your character level in dark damage.

Shifting Darkness- Any creature you reduce to half it's hitpoints (rounded up) or less with dark damage you may choose to teleport it up to 30 ft from where it currently is.

Traits:
Ruler of Dynamis- You become immune to Sleep, Fear, and Blind effects.

Ability Score Increase: +8 Strength, +8 Dexterity, +2 Intelligence

Physical Changes: Skin changing to red or black, growing a demon tail, growing metal blades out of one arm, perpetually evil expression.


Ifrit
Natural Armor: Increase Natural Armor by +2

Fast Healing: Gain fast healing 2.

Senses: Gain low light vision.

Speed: Increase base land speed by +10 ft.

Special Attacks:
Slam- Gain a slam attack that deals 1d8 damage.
Hellfire- All your attacks deal an additional +2 fire damage. Any creature you reduce to 0 hitpoints or less is instantly disintegrated and cannot be healed or revived.

Traits:
Fire Immunity- You are immune to all fire damage.
Intense Flame- You gain a +2 racial bonus on all Sunder attempts. Any creature wearing or wielding an object you successfully sundered takes 1d6 fire damage.

Ability Score Increase: +4 Strength, +4 Dexterity, +4 Constitution, +4 Charisma, +2 Wisdom.

Physical Changes: Red hair, bronzed skin, growing horns, increased muscle size.


Leviathan
Natural Armor: Increase Natural Armor by +6.

Speed: Gain a swim speed of 40 ft. You also gain the Jet monster ability.

Senses: Gain a +6 racial bonus on all perception checks.

Special Attacks:
Bite- Gain a bite attack that deals 2d6 damage. If you deal damage with your bite attack you may attempt a Grapple action as a free action.
Overrun- You may perform Overrun attempts as if you were a creature one size larger than you actually are.

Traits:
Ocean King- You gain a +8 racial bonus on all charisma based skill checks against creatures with the aquatic subtype. Also, you may breathe underwater.
Aquatic Acrobatics- While underwater you are considered to have a constant Freedom of Movement effect.
Great Beast- For the purposes of grappling, special attacks, and the weapons you wield, you are considered to be one size larger than you actually are.

Ability Score Increase: +8 Constitution, +4 Strength, +4 Intelligence, +2 Wisdom.

Physical Changes: Becoming much larger, riffled ears, growing gills, elaborate fish scale patterns.


Odin
Iron Muscles: You gain a bonus to your natural armor equal to 1/2 your strength modifier +1. You may also add your Strength modifier to your Reflex and Fortitude saves in place of Dexterity and Constitution respectively.

Senses: Gain a constant Death Knell effect.

Speed: When charging you may move at 3x your speed and when running you may move at 5x your speed.

Special Attacks:
Zantetsuken- Increase your critical modifier with all weapons by +1.
Cut Through Anything- All weapons you wield are considered to be made of adamantine.

Traits:
Sleipnir- You gain a mount, as per the Cavalier class feature. You are considered a Cavalier of equal level for the purposes of this mount.

Ability Score Increase: +8 Strength, +4 Dexterity, +4 Constitution, +2 Intelligence.

Physical Changes: Grow antlers, your voice becomes naturally loud, your eyes become supernaturally piercing.


Phoenix
Fast Healing: Gain fast healing 4.

Senses: Gain 30 ft of blindsense.

Speed: Grow wings and gain a 30 ft fly speed with perfect maneuverability.

Special Attacks:
Fire Wing- Three times per day you may launch a wave of flame in a 30 ft cone. That flame deals 3d6 fire damage, DC 9+character level Reflex save halves.

Traits:
Phoenix Down- Once per week you may revive yourself or an ally. This restores a dead character who has been dead less than a week to full health.
Holy Flame- Any allies (you choose) caught in your Fire Wing attack are healed by the amount they would normally be damage by.

Ability Score Increase: +6 Dexterity, +4 Charisma, +4 Wisdom, +2 Constitution, +2 Intelligence.

Physical Changes: Multi-colored hair, brief aura of flame when injured, long hawk nose, lengthened limbs.


Ramuh
Perceptive Defense: You may add your Intelligence modifier to your AC in addition to your Dexterity. You may only be wearing light or no armor for this ability to activate. You may also add your Intelligence modifier to your Will saves in addition to Wisdom.

Spell Resistance: Gain spell resistance 10.

Special Attacks:
Judgement Bolt- By spending 1 MP (non-caster classes are considered to have MP equal to their character level for the purposes of this ability), you may attempt to summon a bolt of lightning from the sky. It targets a single creature within 40 ft and deals x2 Intelligence modifier in damage, fortitude halves. You may spend up to 3 additional MP to target an equal number of separate creatures with the same attack.

Traits:
Negate- Any creature you damage with a spell or your Judgement Bolt ability lose all beneficial spell effects, such as Haste or Shell.

Ability Score Increase: +8 Intelligence, +4 Dexterity, +4 Wisdom, +2 Charisma

Physical Changes: None.


Shiva
Natural Armor:Increase Natural Armor by +4.

Senses: Gain 60 ft of darkvision. By spending 1 MP (non-caster classes are considered to have MP equal to their character level for the purposes of this ability), you also gain 10 ft of Truesight for 6 seconds.

Speed: Increase base land speed by +10 ft.

Special Attacks:
Diamond Dust- Any time you succeed on a critical attack against any enemy they take an additional +4 Ice damage and become Frozen for 1d4 rounds.
Axe Kick- You gain the Improved Unarmed Attack feat and the unarmed attack damage as a monk of equal level.

Traits:
Queen of Battle- When selecting bonus feats you count as both a Fighter and Monk of equal level.

Ability Score Increase: +8 Dexterity, +4 Constitution, +2 Intelligence, +2 Charisma, +2 Strength.

Physical Changes: Skin becomes a shade of blue, hair lengthens a considerable amount, physical attractiveness rises considerably.


Titan
Damage Reduction: Gain damage reduction 10/Adamantine.

Senses: Gain 120ft of Tremor Sense.

Speed: You may move at full speed when wearing medium or heavy armor.

Special Attacks:
Slam- Gain a slam attack that deals 1d10 damage.
Awesome Blow- Gain the Awesome Blow feat and you may use it as a creature one size larger than you already are.

Traits:
Rock Throwing- Gain the Rock Throwing special monster ability, which you may use a creature one size larger than yourself. All thrown rocks of at least small size or larger deal 1d12+1/2 your strength modifier in damage.

Strength of Stone- Gain a +4 racial bonus on all Fortitude saves and your CMD. When calculating your CMD you may use your constitution modifier. Become immune to Bleed condition and bleed effects.

Ability Score Increase: +8 Constitution, +6 Strength, +4 Wisdom.

Physical Changes: Becoming wider, skin changes to a rocky texture, grinding noise when you move quickly, cracks appear instead of bloody wounds.


Valefor
Natural Armor: +2 Natural armor bonus.

Senses: Gain 60ft of darkvision.

Speed: Grow wings and gain 40ft of fly speed with perfect maneuverability.

Special Attacks:
Claws- Gain two claw attacks that deal 1d6 damage each.
Eagle Charge- When charging from a greater height than your opponent, you may use the Pounce monster ability.

Traits:
Perfect Flyer- Gain Flyby Attack and Hover as bonus feats.
Born in the Air- Gain a +4 racial bonus on all Reflex saves while flying.

Ability Score Increase: +6 Dexterity, +4 Constitution, +2 Strength, +2 Charisma, +2 Intelligence

Physical Changes: Your hair becomes feathered, you occasionally hover a few inches off the ground, your eyes become black like a birds.
