I'm trying to figure out if there is any printed source that has a conjuration spell similar to this: creates an object inside a person essentially tele-fragging them but rather than killing them in just inflicts damage. With maybe a reflex save to dodge Or create an object the is ethereal until it goes through someone at which point it becomes a physical object causing bodily harm.

Trying to avoid reflavoring other spells though.

The Necrotic Cyst spells are the only thing that comes to mind, and it isn't exactly what you're looking for..

Only ability I know of that does anything like that is psionic, and that just teleports someone's brain stem.

There are a couple of drowning spells that create water inside someone's lungs, and psionic powers that deal damage via garbled teleportation of the target. Otherwise no, tele-fragging is explicitly against the normal laws of magic and they didn't write any spells that contradict it.

If the subject of Death by Thorns succeeds it's save, that is what happens. If it fails, it dies in 1d4 rounds.

The Spell Compendium version of Curse of Lycanthropy is somewhat like this in that it creates wererats within someone's body, but it's Necromancy rather than Conjuration.

Baleful Teleport is a psionic teleportation effect that deals 9d6 damage. Closest I can think of. Might be ported over to a spell, it's not that much damage.

Just remembered something that may rain on your parade:

Note: Conjuration spells can’t create substances or objects within a creature.

Wouldn't the fact that Death by Thorns does exactly that make it a case of specific > generic?

Wouldn't the fact that Death by Thorns does exactly that make it a case of specific > generic?

Possibly.

However, the 3.0 version of Create Water does not include the bit of text on conjuration spells being unable to create matter inside creatures, so Death By Thorns wouldn't have been in violation of the rules when it was released. The subsequent update of Create Water may very well have rendered the spell invalid.

It's Necromancy rather than Conjuration, but Bonefiddle does more or less that. Bonus points for playing someone's skeleton like a macabre violin.
