How does one acquire the monster of legend template? I know a god has to give it to you but what do you have to do for it? Also could the god give himself the template?

A god is generally an outsider and not eligible for the template.
How the template is bestowed isn't really described in much detail, so it's pretty much in the realm of the DM to invent his own fluff...

Any deity with alter reality can do basically whatever it wants to your character, you just have to draw its attention. You want the monster of legend template; tell your DM, get a god's attention, then do something legendary. Be prepared to eat an ad-hoc level adjustment if your DM says yes.

Literally be the stuff of legends. And propagate like a mofo. In 2-3 centuries, you'll be a legend: erego, free template.
Alternatively, be the first person of your particular race.

The template states that it can only be applied to an animal, magical beast, or monstrous humanoid.

The second is probably easier to achieve: just travel back in time a bit.

They can be if they were a monster of legend before turning undead, but they can't become one after turning undead, no.
