Me, I remember picturing the Balrog as basically humanoid in form. Massive, powerful-but-gaunt humanoid shadow surrounded by dark fire. I don't have Fellowship to hand - I mention because embarassing slip-ups can cement themselves into my head-pictures very easily - but I gotta tell you, on this account, the rawr-demon Peter Jackson interpretation of the Balrog did not do it for me.
On the other hand, I absolutely did picture Sauron as a giant eyeball, and was actually quite jolted when Gollum mentioned he had a hand.
