There are several things that can cause you to be teleported to a random plane of existence, like prismatic wall, or possibly when moving a sphere of anhililation through a portal.
My question is how to determine that random plane. Is there a table I just haven't found or does the dm just pick one?

Just pick whatever sounds fun to DM.

There is the "Astral Color Pools" and "Ethereal Curtains" tables in the DMG.

01 Ysgard
02 Limbo
03 Pandemonium
04 The Abyss
05 Carceri
06 Hades
07 Gehenna
08 The Nine Hells
09 Acheron
10 Mechanus
11 Arcadia
12 Mount Celestia
13 Bytopia
14 Elysium
15 The Beastlands
16 Arborea
17 The Outlands
18 Ethereal Plane
19-20 Material Plane
01 Material Plane
02 Shadowfell
03 Feywild
04 Plane of Air
05 Plane of Earth
06 Plane of Fire
07 Plane of Water
08 Elemental Chaos
Maybe use whichever seems more appropriate? Or combine them into a single big table.

I wouldn't have put the Elemental Chaos in there. It seems to mean instant death, like the Positive and Negative planes would.
Edit: I forgot that an ethereal curtain sends you to the Border Ethereal of a plane.

Places you could end up in 5e... places marked with ? Are old edition fluff that hasn't yet been contradicted

The Prime
-Another Campaign world
-Wildspace

Transitive Planes
-Ethereal
-Astral
-?Deep Shadow?

Inner Planes
-Elemental Planes
-Elemental Chaos

Outer Planes:
-any of the 17, many with multiple layers to choose from

The convergent Planes
-Feywild
-Shadowfel

Other places
-positive energy
-negative energy
-Far Realm
-?temporal energy plane?
-?realm of dream?
-?mirror plane?
-a variety of demiplanes

Probably could draw up an easy chart based on how much you want them to survive such a trip

Pick your 4, 6, 8, 10, 12, or 20 favorites, and roll for it.

Better yet, make them roll for it.

Thanks for reading,

Thank you so much, this is really awesome! If this ever comes up I'll be sure to let you know!I am glad you like it. It was put together rather quickly, but I did try to think it over. For example, I know the probability of arriving in some version of the Ethereal Plane is one in three, and I think that makes sense.

It also made me realise that one my peeves with the new cosmology, that you can't explore the Elemental Chaos anymore, isn't entirely correct. You can, sort of, if you are in the Border Ethereal.

I agree (though that 'map' is confusing because nothing in the text ties them to the outer planes or any alignment orientation). 1e had the energy planes aligned this way, positive and negative with the Prime Material 'between' them; separate from both the inner and outer planes. The temporal energy plane exists in similar separation

I'm not sure if it is explicit, but doesn't it seem like the feywild and shadowfel are transitive planes between the Prime and the energy planes now? Like... they feywild is 'positive energy infused' and the shadowfel is 'negative energy infused'? In my headcanon anyways it would be possible to reach the energy planes through extensive travel in the feywild/shadowfel

Like... they feywild is 'positive energy infused' and the shadowfel is 'negative energy infused'? In my headcanon anyways it would be possible to reach the energy planes through extensive travel in the feywild/shadowfelIt would match how they were described in Nentir Vale. When the Primordials made the world, they did not like it having regions that were "too bright" or "too dark", so they pushed those to the sides and that's how the world got its two echoes.

Also, in 5e, all the Inner Planes save the Elemental Chaos are divided between an "innermost" region that ressembles the Material Plane, so it feels like being in a world with a dominant element, and an "outermost" region that is just one big expanse of a given element. It would have been easy to describe the Feywild as the innermost region of the Positive Plane, and the Shadowfell as the innermost region of the Negative Plane.

Alternatively, I had an idea for 5e Planescape / Spelljammer where the innermost / outermost regions would simply be how worlds and outer space look like in the various planes. And there would be crystal spheres floating in the Elemental Chaos, with the so-called Material Plane inside of them.http://i.imgur.com/5pFH0bF.png But further discussions on the planes made me realize it might be too much of a departure from how a D&D cosmology is expected to work.

I'm sure there is an official chart, somewhere...
