This is a starting list of all the classes that get Evasion as a class feature as well as various items, feats, prestige classes, and miscellaneous effects that use it. Mounts, familiars, and creatures in general are not included in this.

Base classes that grant Evasion:

Monk (Player's Handbook page 39), 2nd level
Rogue (Player's Handbook page 49), 2nd level

Whirling Frenzy Barbarian (Unearthed Arcana page 66), 14th level, gain Evasion instead of Indomitable Will during Whirling Frenzy

Incarnate (Magic of Incarnum) can get evasion by binding Impulse Boots to his Feet Chakra
Totemist (Magic of Incarnum) can get evasion by binding Great Raptor Mask to his Totem Chakra. They can also grant their mount evasion by binding Riding Bracers to their Totem Chakra.

Divine Oracle's Prescient Sense (Complete Divine page 34), 2nd level, specifically says "This form of evasion works no matter what armor the divine oracle wears, unlike the evasion ability used by monks and rogues." Depending on your DM this might count as Evasion for all other purposes.

Master Specialist's Moderate School Esoterica (Complete Mage page 70) 7th level, only if specializing in Abjuration, only after casting an Abjuration spell, and it only works against actual spells (not EX, SU, or SLAs). Effectively Evasion (and Mettle), but isn't named as such.


Prestige Classes that grant Improved Evasion:

Water Cloak (Magic Item Compendium page 148), gain Evasion against any applicable effect that deals fire damage.
Ring of Evasion (Dungeon Master's Guide page 232), gain the Evasion ability.

Crimson Ruination Legacy Weapon (Weapons of Legacy page 61) 14 HD, gain Evasion against dragon breath weapons.
Legacy Item ability (Weapons of Legacy pages 179-203), gain Evasion from Ability Menu F.

Boots of Swiftness (Epic Level Handbook page 144), Epic boots that grant Evasion and other bonuses.

Book of Warding - minor artifact in Dragon #325 - works like a usual Wizard's spellbook; contains all spells of Abjuration school, and Divination (scrying) subschool; the first wizard in 24 hours who prepare spells from it - gets some additional benefits (for 24 hours); the most common benefit is force shield (+4 shield bonus to AC, and magic missile immunity); if Wizard prepared at least 82 levels of spells, he will also get Evasion

Spells that grant Evasion:

Aura if Evasion (Spell Compendium page 18), cleric 5, sorcerer/wizard 6 spell that grants you and all creatures within 10 feet of you evasion against breath weapons.
Nightstalker's Transformation (Complete Adventurer page 155, Spell Compendium page 148), sorcerer/wizard 5 spell grants Evasion and other things.

Friendly Fire Evasion Teamwork Benefit (Heroes of Battle page 17), gives allies evasion with respect to allied spells being cast.

Aglarondan Griffonrider (Unapproachable East)

Aerial Evasion (Ex): Beginning at 2nd level, when an Aglarondan griffonrider is airborne on her bonded mount, both she and her mount gain the benefit of evasion.

Spelldancer 2 gives evasion as well, notably with full advancement of spellcasting.

Incarnate can get evasion from binding Impulse Boots to his Feet Chakra.

Just like standard Rogues, a Psychic Rogue gets evasion at level 2 and can take Improved Evasion as one of his special abilities (at level 11, 14, 17 or 20).

It is missing Initiate of the Draconic Mysteries [Drac. 133]

The only class I am aware of that receives Evasion on 1st level!

You might have missed Shade Hunter (Champions of Ruin p.58)? At 6th, they get to chose one of the following Rogue special abilities: defensive roll, improved evasion or skill mastery.

Infinite Variety - capstone of Master Transmogrifist PrC - allows during casting Polymorph, PAO, or Shapechange to add SQs from different creatures to resulting form. This way, you may get Evasion from Ectoplasmic Vermin, or Improved Evasion - from Psicrystal (http://www.d20srd.org/srd/psionic/monsters/psicrystal.htm)
