If an item casts fly once a day does that mean you have to concentrate on it like you cast it ?

Yep. But with most items that allow casting I usually give advantage on the save, because I'm not fond of the way this works.

Only if it says "you cast", then you have to maintain the concentration. If not, then no concentration is needed. That's how we play it at our table. I can't recall of this is RAW.

DMG p141. Casting a concentration spell from an item, such as fly from an instrument of the bardo, requires concentration.

I can't imagine casting fly nowadays with the concentration rules the way they are, too dangerous, especially if you want to fly higher than 10 feet.

That why you also memorize feather fall :)

To be honest I’m not a fan of the concentration rules in 5e. We have our own house rules.

1) Only roll concentration checks if below 50% hp or hit by a critical
2) Concentrate on more than one spell based on level 1st-7th ( 1 conc), 8-15 (2 conc, based on highest level pf spell you able to cast (IE 8th level caster can cast a 4th level spell, thus he can maintain a 3rd and 1st level spell or two 2nd level or just one 4th level spell) 16 – 20 (3 conc)

Oddly enough this just came up in the podcast of dragontalk. Jeremy Crawford on there said "you do have to concentrate unless the item says otherwise"
