My local hobby store has just started getting serious about stocking Reaper Bones, and among other things I had to pick up the Tiik Baron (http://www.reapermini.com/OnlineStore/tiik/sku-down/77212).

I like it, but I have no idea what this is supposed to be in game terms. Is this based on any specific creature, and if so does it have stats in 3.5 or Pathfinder?

The closest fit for 3.5 would probably be a mutant 4 armed Sahuagin (http://www.d20srd.org/srd/monsters/sahuagin.htm#sahuaginMutants).

Here's the WotC mini for comparison:

Okay, thanks. Turns out that Pathfinder has something very similar, the sahuagin prince from the Monster Codex.

In either 3.5 or Pathfinder, is there a four-armed template, or some other way to get those extra two arms?

The Insectile template (Savage Species, p.121) gives extra limbs, although they aren't usable for much.

Sahuagin have an inbuilt mutation that means some of them have 4 arms to begin with.

The miniature also vaguely resembles a Mezzoloth (MM3):

Aha, this explains everything. Thank you, I was forgetting that.

On a related note, I've been wanting to work up a four-armed warforged for years, but wasn't sure how to add the extra arms. Maybe just a bonus feat for Multiattack?

If you can d20 modern has a neat mutation chart that's fairly balanced that could get you extra arms.
I also recall there being some variant half fiends that gave extra arms (half marlith I think)
