Putting some spaces between the questions would really help here. Jeez.

But to try to answer:

A.2

B.2

C.1

D.1

E.5 (Other - Depends on the campaign)

F.2

G.5 (Other - All races in PHB, SCAG, and Volo's)

H.5 (Same as G, plus UA allowed)

I.5 (Other - pre-made campaigns modified for the group)

I'm currently a part of 3 games (play in two, dm one) so I'll answer the questions for all 3 (G1 is dm'd by a good friend, G2 is by an acquaintance, G3 is by me)

A. Does the DM follow the game rules?
All three: 2 - Most of the time, with a few house rules (different in each game)

B. Campaign World
G1: 2 - Modified Forgotten Realms, G2: 4 - Completely homebrew, G3: 4 - Completely homebrew

C. Gods
G1: 1-2 - Forgotten Realms + Greyhawk deities, G2: 4 - Only homebrew, G3 - 2 - Gods from all major campaign settings allowed, Greyhawk encouraged

D (Your number scheme gets a bit off here). Heroes/Villains
All three: 4 - Completely homebrew

E. Starting level
G1: 1 - First level (but I joined late at third level), G2: 2 - Third level, G3: 1 - First level (I'd normally do 3rd or 5th level, but they were all new players)

F. Ending level
G1: Implied 15th, G2: Unknown, G3: Unknown

G. Races (you might want to include options for elemental evil, ua, and volos)
All three: 2 - PHB + exotic PHB

H. Classes
G1: 1 - PHB (No UA, including revised ranger), G2: Any from PHB, magic users discouraged. UA is allowed with permission. G3: PHB, only revised ranger from UA

I. Pre-made adventures
G1: Mostly homebrew, will pull some oneshot prewritten adventurers, G2: Homebrew, G3: Homebrew

J. Magic Item use
G1: Incredibly generous, G2. Somewhat generous with a few homebrew magic items, G3: Somewhat generous, almost exclusively homebrew magic items

In the games I run...

A: 2

B: 4, I have drawn my own maps and so far haven't taken inspiration from any published setting for regions or cities.

C: 4

D: 4, but my setting is relatively low magic, so there aren't many heroes just walking around. Most of them are mythic or legendary Heroes remembered only in stories now. Same for villains.

E: 1, usually at second level and they will advance to third level quite quickly. I start at first level of I want the characters to have more of a chance to role-play the development and growth of their character.

F: 2

G: 2, but I don't use all the exotic races.

H: 1, but this depends on what nation you're in. Some places have no monasteries and no-one practices martial arts much, but another has its royal guard as only the elite martial artists.

I: 4

J: 4, but I'm not very generous. Magic items are rare and powerful. Mostly, the players don't have the social status to just buy them, so need to resort to robbery, black market trading or go and research where one might be left somewhere or else gain status so they can buy one. His doesn't go for some consumables.

G0-G2. Based on modules generally
Somewhat generous - very few items are ever bought apart from scrolls/learning wizard spells and rarely healing potions.
Including Item customization per the DMG - quirks etc

Game 1
A.Does your DM follow the game rules
2. Most of the time with less house rules
3. Somewhat, with more house rules

A mix between the two. When it comes to skill checks it is 3, verging on 4. The skill requirements have arbitrary DCs, which annoys me to no end.

B. Is your campaign world
5. Completely homebrew with no ties to the above

C. Gods in your realm
4. Only home brew gods

D. Same question as 3, but with Heroes
4. Only home brew heroes (Assuming you mean same question as C)

5. Same question as 3, but with villains
4. Only home brew villains (Assuming you mean same question as C)

E. What level do you start at when you make a new campaign
2. Third level

F. What level does your campaign usually end
3. 11-15th level

Although some changes have happened recently, which leads me to believe there will be a short campaign option finishing at ~7th.

G. Race used in your game
4. All races and home brew races

H. Same as G but with classes
4. Classes are all published, but certain abilities and classes have been cut.

I. Pre made Adventures d&d
4. All home brew

J. Magic item used
5. You're lucky to have 1 magic item, and even more lucky if that item suits you.

Game 2
A.Does your DM follow the game rules
1. All the time

Allows some bending for rule of cool.

B. Is your campaign world
4. Completely different home brew world with non or minor above realms or major cities

Homebrew with elements of APs added.

C. Gods in your realm
5. Published gods available regardless of realm.

D. Same question as 3, but with Heroes
4. Only home brew heroes (Assuming you mean same question as C)

5. Same question as 3, but with villains
4. Only home brew villains (Assuming you mean same question as C)

E. What level do you start at when you make a new campaign
1. First level or second level

F. What level does your campaign usually end
3. 11-15th level
4. 15-20th level

Haven't finished one yet, but we're aiming to finish around mid-teens.

G. Race used in your game
2. Standard players handbook plus exotic players handbook 1/2 orc, Drow, Gnome, tiefling Dragonborn

Some restrictions apply.

H. Same as G but with classes
2. Standard players handbook plus exotic players handbook

I. Pre made Adventures d&d
2. Pre made used in whatever world used by group

J. Magic item used
2. Somewhat generous
4. Adding home brew magic items

These are not mutually exclusive, so both are true.

It varies, but my two current campaigns;

A: 2
B: 4
C, D, ?5?: 4
E: 1
F: 4
G: 2+Volos/EEPG/etc; no flying races; fluff often heavily modified
H: ?2? (All official publishes material with very limited UA access)
I: One current campaign 3; but generally... 1 or less in 5e so far? - probably 50% homebrew items

A.Does your DM follow the game rules -
House rules are still rules, right? This isn't asking whether the DM follows their own rules, but specifically the rules in the book, I'm assuming. This could probably be reworded to something like "How close to the RAW do you play/run your games?" or "How many homebrew rules do you use?"

1. (All the time)only RAW
2. (Most of the time with fewer house rules)/few or minor house rules
3. (Somewhat, with more house rules)/ several or major house rules
4. (Rarely, with predominately house rules)/ almost entirely house rules

A: 3. - major modifications to initiative and combat turn procedure and a few other things

B. Is your campaign world
4. Completely different home brew world.

C. Gods in your realm
4. Only home brew gods

D1. Heroes of your Realm
4. Only home brew Heroes

D2. Villains of your realm
4. Only home brew Villains

E. What level do you start at when you make a new campaign: true answer actually depends, the choice isn't universal across all games, ever, but generally -
1. First level or second level

F. What level does your campaign usually end
2. 7-10th level - caveat: this isn't normally a premeditated choice, but just how life usually goes. An end to a campaign is usually not planned at all.

G. Race used in your game
1. Standard players handbook -most accurate answer is 1.5* - standard races with some homebrew, maybe one homebrew race replacing one standard.

H. Classes used in your game
1. Standard players handbook - more accurate: some of the PHB classes, with some modifications

I. Pre made Adventures d&d
4. All home brew - more accurate answer 3.5*- Mostly home brew sometimes with cannibalized bits of published adventures (never a complete adventure run as it was published)

J. Magic item used
1. Based on DM guide and level + 4. Adding home brew magic items

Some of the choices in some categories don't seem to be answers to the same question.

On magic items, whether or not you include home brew items is not connected to how many magic items you tend to give out. There should also have been an option if you use less than what the DMG would give. The amount of homebrew items is a separate question.

On classes, there is no option for a more restricted selection of the PHB classes versus all of them.

I'm answering for when I DM (which is the norm for our group, although currently someone else is running CoS for us).

A.Does your DM follow the game rules
1. All the time
2. Most of the time with less house rules <- It's more likely to be on-the-fly rulings for situations where the RAW don't make "sense" than formal house rules.
3. Somewhat, with more house rules
4. Rarely, with mostly house rules

B. Is your campaign world
1. Forgotten Realms, Greyhawk, Eberon, Dragon lance with no world house rule changes <- We play in The Known World (the default setting of the B/X and BECMI editions), but this is the closest of the given options.
2. One of the above worlds with few world house rules
3. A Home brew world with the above worlds ( all or some of the realms combined on 1 world) each it's own continent on 1 world.
4. Completely different home brew world with non or minor above realms or major cities

C. Gods in your realm
1. Specific to the major realm your using
2. More towards that realm, but a few extra from other major worlds or home brew <- None of the options quite fit, but this is the closest. We use real-world pantheons.
3. Less towards that world or other major realms and more towards home brew.
4. Only home brew gods

D. Same question as 3, but with Heroes
1. Specific to the major realm your using
2. More towards that realm, but a few extra from other major worlds or home brew
3. Less towards that world or other major realms and more towards home brew. <- The canonical NPCs of the setting theoretically exist, but I avoid having them appear in games, so I don't really "use" them.
4. Only home brew heroes

5. Same question as 3, but with villains
4. Only home brew villains

E. What level do you start at when you make a new campaign
1. First level or second level <- Always first level
2. Third level
3. Fith level
4. Higher than 5ifth level

F. What level does your campaign usually end
1. 6th level or below
2. 7-10th level
3. 11-15th level
4. 15-20th level <- Sometimes higher than 20th, using a 5e conversion of the BECMI "Immortals" rules.

G. Race used in your game
1. Standard players handbook
2. Standard players handbook plus exotic players handbook 1/2 orc, Drow, Gnome, tiefling Dragonborn <- This, plus exotic races from the EE player's guide and Volo's (but not world-specific ones).
3. Standard with exotic from that world Such as Eberon, Dragonlance
4. All races and home brew races

H. Same as G but with classes
1. Standard players handbook <- This
2. Standard players handbook plus exotic players handbook
3. Standard with exotic from that world Such as Eberon, Dragonlance
4. All classes and home brew classes

I. Pre made Adventures d&d
1. Only according to the corresponding world for the pre made <- I went for this, but only because there was no "all of the above" option.
2. Pre made used in whatever world used by group
3. Rarely use pre made and more home brew
4. All home brew

J. Magic item used
1. Based on DM guide and level
2. Somewhat generous <- On the one hand I don't give out more wealth than expected, but on the other hand I let PCs buy magic items of their choice using that wealth; so it kind of evens out to this.
3. Very generous
4. Adding home brew magic items

A.2. I am the DM and I try to stick to the rules, but sometimes we forget something or we simple find some ruling better/easier/more reasonable and we change the original.
B.1 Forgotten. It si much easier to follow the pre-gen adventures. Plus I like Forgotten Realms. Of course there are hose rules but they are so minor that I think I fall in the category B.1. anyway.
C.1. The Gods are the ones from the Player's Handbook. Just once a player made up a new god and we are using it, but it is mostly as a joke.
D1.2. The Heroes in my campaign are mostly cameos (except for the PCs of course). They are of two type: very famous Marysues (e.g. Dri'zzt) or old PCs of my players that have become famous.
D2. ? Villains tend to be short-lived, so they are new in every Adventure. I don't really know how to answer this one. They are almost always from pre-gen so maybe 1?
E.1. Using pre-gen I start when they tell you to start: level 1!
F.3. Campaign ends between 11-15th for the same reasons as before.
G.2. Even if I try hard to suggest new rulebooks, my players always use only the Player's Handbook.
H.1 Only Player's Hanbook classes.
I.1. Pre made played in Forgotten!
J.2. I think I am somewhat generous. I also use a lot of Homebrew made up from me or found on the Internet.

Survey! That's really hard to read though. :smallfrown:

A. 2? That's me rating myself as a DM, by the way.

B. 2. I usually use FR with minor changes.

C. 1. Having pre-written pantheons is one of the reasons why I like FR.

D. Not sure what you mean? The PCs are the heroes.

E. Depends on the campaign. My current players started at level 2.

F. 1. Games end when they end, but few get above 6th.

G. 4. I often use homebrew races.

H. 3. I'll generally allow a wide range of class options.

I. I've never run a pre-made module.

J. 2. I feel like I'm somewhat generous.

JackPhoenix

Note that the answers are for the current game(s). The answers may be different in a different campaign, obviously.

A.Does your DM follow the game rules: 2. Most of the time with fewer house rules

B. Is your campaign world: that depends what do you consider "world house rule changes". I play Eberron, where you're supposed to do some customisation

C. Gods in your realm: 1. Specific to the major realm your using

D1. Heroes of your Realm: What does that mean? PC's are PC's, NPC's exist, obviously

D2. Villains of your realm: 3. Less towards that world or other major realms and more towards home brew.

E. What level do you start at when you make a new campaign: 2. Third level

F. What level does your campaign usually end: Inconsistent. I've had campaigns running to teens, I've had campaigns ending before level 5 (mostly because RL issues forced us to stop playing for a while, though). The goal is 3. 11-15th level

G. Race used in your game: Neither. I may allow some homebrew races while banning some core races (dragonborn come to mind). So somewhere between 3 and 4

H. Classes used in your game: Once again, neither. Certainly not "all homebrew", but closest to 4

I. Pre made Adventures d&d: neither. Running homebrew games while stealing being inspired by interesting bits from published adventures

J. Magic item used: 4 isn't exclusive with the other choices. 4 and 2.

A.Does your DM follow the game rules: 3. Somewhat, with more house rules

B. Is your campaign world: neither. It's none of the settings mentioned in the first answer, but neither is it (complete) homebrew.

C. Gods in your realm: What gods?

D1. Heroes of your Realm: again, what is that supposed to mean?

D2. Villains of your realm: 4. Only home brew Villans. At least so far.

E. What level do you start at when you make a new campaign: 2. Third level

F. What level does your campaign usually end: it's the GM's first game, and it's still running

G. Race used in your game: neither. Closest to 3, but some standard races do not exist (halflings) and most of them had been modified. And homebrew races for the world

H. Classes used in your game: neither. Closest to 3, same as with races. Druids and warlocks do not exist in the D&D form, sorcerers do not exists at all.

I. Pre made Adventures d&d: 4. All home brew

J. Magic item used: none so far. We're still low level, but the GM certainly isn't using DMG guidelines (mostly because he's propably not even aware of them)

Honestly, seeing as most of my answers were "neither", I think the quiz isn't very well done, because it lacks "in between" options and some questions aren't clear.
