I wil start my question with heals

When a player takes physical damage from a spell, or melee damage does a healing spell or potion removes any scar from that injury. Thus you never have scars

Back in the high school times for fun we had a hitting chart when a 20 was rolled vs us or enemy.
If the damage did a third of your hit points you received a scar, if a 20/19 happened and you lost 1/2 of your hit points you lost a piece of the body. Fingers, eye hand foot, nice etc.

20/20 was instant kill for anyone.

Anyone use hit charts and what's your rule on scars and healing if any

No charts for 5e. And generally no scars unless requested by the player. My players would be some gruesome basterds, they have been fighting various age range black dragons recently. Lotta Acid damage to the face.

Up to the DM. In the standard forgotten realms, it has always been based on which deity provides the healing magic. Some left scars and some did not. Healing potions have never been specified either way so it's up to the DM.

Note that hit points are abstract concept and not every "hit" actually hits hard enough to cause wound (or at all).
