I remember there being an NPC who was faceless but had a spirit of some sort that covered her face to give herself a look. Anyone know what i am speaking about? If so, where could i find her?

That would be the Symbol (CR 20), a villain from the Cityscape supplement (p. 122); a genderless unholy scion, with levels of Dread Necromancer and a bunch of other goodies.

The "Mother" is the Ghostly Visage familiar the Symbol partnered up with.

I've used the Symbol a few times in my campaigns; the second time, s/he wiped out our party. It was awesome.
