Quick 3.5 rules Q&A question here:

Do Changelings instinctively know when they're dealing with other Changelings?

Like, an Elf meets a human, but they're both Changelings. Can they tell? Are there Changeling pheromones or something?

No? I don't know why there would be. If one fails a disguise check then the other night realize but there's no rule for them knowing.

Change shape effectively disguises you as the race you're pretending to be, there's no disguise/spot opposed checks. For changelings, that would only come into account if you're trying to disguise yourself as a specific individual.
